Description Swedish Health Services is looking for a Social Work
Case Manager (1.0 FTE, Day Shif t) to work in the Behavioral Health
Partial Hospitalization Program at Swedish Edmonds in Edmonds, WA.

Benefits! SCHEDULE: 40hrs; 5 days per week/8 hour shifts.
Monday-Sunday; rotating weekends. 8-4:30pm PST. Can choose schedule
to support Intpatient Psychiatric Unit or Partial Hospitalization
Day Program. Crosstraining is AVAILABLE. The Social Work Case
Manager provides clinical case management functions via a
collaborative process that transitions patients to optimal clinical
outcomes while assuring safe, cost-effective care. Demonstrates a
daily commitment to the department's value of teamwork. In this
position you will have the following responsibilities:
- Psychosocial Essential Functions:
- Conducts psychosocial need assessments for patients and
families during hospital stay, identifying risk factors and
strengths of patient/family support system
- Provides supportive counseling and crisis intervention,
evaluate patient needs for psychiatric referral and provides drug
abuse assessments. Facilitates referrals for
post-discharge/extended needs.
- Facilitates resolution of all legal issues including:
protection; adoptions; administrative, police and CPS holds; power
of attorney; guardianship etc. Completes mandated legal reporting.
Conducts domestic violence assessments and assists with safety
plans.
- Consults with patients/family members re: advance directives,
POLST etc. Ensures advance directives are in place and honored
according to the patient's wishes. Assures all ethical situations
are identified and makes referrals to Medical Ethics
Committee.
- Assists with assuring palliative care protocols and
interventions.
- Transition (discharge) Planning Essential Functions:
- Creates and implements transition plans for all patients.
Educates patient and family of transition plans upon admission and
provides updates and/or changes throughout the hospital stay.
Assists patients and families in accessing community
resources.
- Participates in multidisciplinary transition planning meetings
and facilitates patient care conferences.
- Contacts and arranges placements to skilled nursing facilities,
rehab, long-term care, specialty care, home health, etc. to assure
post hospital care. Assures the acquisition of DME, medications and
other discharge needs. Following transition, calls patient/family
as needed to assure appropriate transition occurred.
- Coordinates necessary funding for post-transition placements
and other plans to achieve optimal outcomes.
- Delegates functions to the Case Management Assistant as
appropriate.
- Financial Reimbursement and Utilization Management Essential
Functions:
- Works with Patient Access Liaisons to facilitate screening for
financial needs. Collaborates with the health care team to identify
issues or problems with finances. Helps facilitate applications for
benefits as needed. Collaborates with the health care team to
communicate and co-ordinate with insurance companies to arrange
appropriate post-hospital care and services.
- Maintains knowledge of patients' reimbursement processes and
communicates reimbursement information to patients, families, and
the healthcare team.
- Maintains awareness of relevant managed care contract
requirements and Medicare and Medicaid reimbursements and
guidelines.
- Clinical Coordination Essential Functions:
- Participates in case management and unit healthcare teams.
Collaborates with RN Case Manager, physicians and other team
members. Identifies the "Captain of the Ship" in complex cases with
multiple doctors.
- Communicates findings and care recommendations effectively to
ensure plan of care is being followed for all patients. Provides
consultation and mediation services to facilitate patient
progression towards desired outcomes.
- Maintains credibility as a clinical resource/professional role
model. Ensures consistent psychosocial standards of care.
- Performance Improvement Essential Functions:
- Promotes heath care outcomes in concert with currently accepted
clinical practice guidelines and established clinical
pathways.
- Recognizes and records avoidable delays and avoidable days.
Identifies poor outcomes/data for referral for performance
improvement. Works to remove barriers that impede optimal patient
care.
- Records and appropriately manages patient and family concerns
and complaints promptly.
- Ensures compliance with regulatory standards of care at both
the state and federal levels.
- Cultural and Spiritual Essential Functions:
- Assures patients, families and staff support the cultural
aspects in the delivery of health care. Assures availability of
interpreter services as appropriate to patient/family needs.
- Identifies patient and family support needs for life
threatening diseases, end of life, and bereavement and makes
appropriate referrals.
- Identifies patients' needs for spiritual care and support, and
refer to appropriate personnel. Applicants that meet qualifications

- Master's Degree in Social Work
- Licensed Social Work Associate - Independent Clinical (LSWAIC)
Preferred qualifications for this position include:
- One (1) year directly related work experience social services
in a heath care setting
- Experience in crisis intervention
- Licensure as an Independent Clinical Social Worker (LICSW) with
the Washington State Department of Health under Chapter 18.225 RCW
is preferred and may be substituted
