for professional licensure Job Summary ICHS’ behavioral health
services are collaborative, patient centered, and designed to
improve and enhance patient’s emotional, mental, and behavioral
health. As a member of the patient’s care team, ICHS’s Behavioral
Health program is accountable for the behavioral health care of all
ICHS patients through direct referral, indirect review and
implementation of guidelines and workflows impacted by the
patient’s behavioral health. Patients access behavioral health
services as a member of their care team. The Behavioral Health Care
Coordinator functions as a member of the collaborative care team
that includes the patient, primary care provider/dental provider,
behavioral health provider, consulting psychiatrist, and other
medical staff. This temporary position is responsible for providing
integrated behavioral health care within a primary care setting.
This position is non-clinical and does not provide therapy nor
counseling. This position plans, organizes, and coordinates
behavioral health services in conjunction with other members of the
patient’s care team and identified external providers who impact
patient care, including support with care gap metrics for
behavioral health. This position works collaboratively other
members of the patient’s care team to improve the health of
patients with chronic behavioral health conditions and/or complex
needs; educates patients and families to help them manage their
behavioral health needs; facilitates communication, coordinates
services, addresses barriers, and promotes optimal allocation of
resources while balancing clinical quality and cost management.
Education - Bachelor’s degree in a mental health field (social
work, psychology, counseling, or related field). Experience - One
(1) year experience working with individuals with co-occurring
mental health and/or substance use disorders; individuals with a
disability; individuals of a vulnerable population (elderly,
mothers, children) and/or other high risk groups preferred.
Experience coordinating care, case management, and
developing/maintaining patient resources/contacts with other
agencies/organizations required. Experience in motivational
interviewing and/or experience working with people experiencing
crisis is preferred. Other Requirement(s) - Suicide assessment,
treatment, and management training required by WA State Department
of Health within sixty (60) days of hire.
